Your First Visit

Prior to seeing the doctor, all patients will need to complete their New Patient Paperwork.  This paperwork may be downloaded and printed from this website under the "PATIENT FORMS" section should you like to complete it prior to arriving at our office.  Otherwise, upon your arrival (15 minutes prior to scheduled appointment time), one of our team members will provide you the paperwork to be completed in our office.  When available, we ask that you bring copies of any x-rays, MRI's, lab work, etc.  Also, we will need a copy of your valid Driver's License and Insurance ID (if applicable).

Upon going to a treatment room, our team will talk to you about the reason for your visit and obtain your vital signs.  Your doctor will then evaluate you and discuss their treatment recommendations.


Insurance 

Our doctors are participating providers with the following insurance companies:  Blue Cross Blue Shield, Blue Advantage, VIVA Health, VIVA Medicare Plus and AETNA.

For our Medicare patients, we are non-participating and do not accept assignment.  This means that charges are due at the time that services are rendered and any monies paid by Medicare will be reimbursed to you directly. 

For all other insurance companies we are considered an "Out of Network Provider". We will file ALL insurance claims for you, no need to submit any forms to your insurance company from us!

Some of our services, like Nutrition, are not covered by insurance.

Payment

Payment for services is expected at the time services are rendered.  We accept the following methods:

VISA, Mastercard, Discover, American Express, Cash or Check (with valid photo ID).
